What is the worst piece of design ever done?

So far the infinitely evil plastic clamshell packaging leads the long list of terrible designs over @ --Really Bad Design @ Quora

My personal favorite? The pull handle on the push door.

Many doors in public places have hinges that allow them to stay open if pushed past their normal arc of travel (they're called torsion hinges). In those instances, handles are needed to close the doors again from the non-swing side.

And the corollary: a whole row of doors, but only one of them is unlocked.

Very good one, Deborah, but that's not design, it's lazy and evil janitors who'd rather snicker at the sight of hapless users in search of the unlocked door yanking away at a row of handles than put up a sign saying "This door unlocked".
